Indiana State Law requires all students attending school to have complete and satisfactory immunization records on file at school. All sixth grade students are required to have a Tdap and meningitis vaccine, and twelfth grade students are required to have a second meningitis vaccine. Letters were mailed the week of November 7 to parents whose children have incomplete immunization records. The letter, linked HERE, explained that as of November 28, 2016, the student will be excluded from school and the absence will remain unexcused until proper immunization documentation is received.
Indiana Code (IC 20-34-4-5) states that a child is not permitted to attend school beyond the first day without furnishing a written record, unless:
• The school gives a waiver (for a period not to exceed 20 days); or
• The local health department or a physician determines that the child’s immunizations have been delayed due to extreme circumstances and the required immunization will not be complete by the first day of school. The parent must furnish a written statement and a time schedule approved by a physician or health department; or
• A medical or religious exemption is on file.

The immunization record must include the month, day and year for each immunization. A complete list of required immunizations can be found on the Health Services website page linked HERE.
